The Transition Care Program is currently A 12 bed program; 6 beds in Residential Care (Pyramid Res Facility & Dija Meta) and 6 people can recieve therapy in their own home. Domestic assisstance & personal care can be provided through other services. Clients are not excluded from TCP if experiencing financial hardship. Must be in hospital to be refered.
