Community Nursing,Allied Health (Physio, OT,Podiatry) Day Respite Centre, Social Support & 60 CACPS PackagesSpecialties include gerontology; stomal therapy; diabetic management; wound management; palliative care; medication assistance; continence advice; personal care and Veterans Home Care program. Service is seven days per week from 6.30 am to 3.30 pm. Target group includes Home and Community Care clients, Veterans Affairs clients, post acute, early discharge, privately funded clients and palliative care clients. Service is for all age groups HACC or non HACC eligible.

Spiritus Care Services - Bayside
Spiritus Care Services (7am - 8pm)provides a range of caring services consistent with their mission of enabling individuals to access nursing and related care in the comfort, safety and security of their own home. Clinical nurses, physiotherapists, personal care workers, respite carers and volunteers provide the care, supported by a team of management and education personnel. Specialty services include Mobile Acute Care Service (MACS), continence management, dietician, wound consultant, podiatry program and services for various stages of dementia for clients and their carers. Currently have Leg Club running at Bayside Branch - Friday morning with registered nurses on site. Run a Palliative Care Course for Carers of Paliative patients for 6 weeks at a time call Palliative Care Porject Officer for info. Chronic conditions self-management course for 2hr sessions over 6 weeks call office for info.

Blue Care - Southside Centre Brisbane Region
Blue Care services are targeted at people who are frail aged or younger disabled. Other services include Pastoral care and counselling, clinic facilities at West End, staff development and education, palliative care, wound management, diabetes management, continence service, volunteer services, Veterans Home Care, Community Aged Care Packages, respite for carers funding, personal care and social support services to people who live in boarding houses and supported accommadation. All services based on assessed need & agreed care plan. Majority of services delivered between 7am-4pm. Essential nursing care may be provided on week-ends & evenings. HACC Eligibility applies - if not HACC eligible may explore other funding source or pay fee for service. Fee schedule applies for private [non funded] clients. Referral via Central Intake Unit 3722 1740 - Doctor's referral required for medical treatment
