The Transition Care Program is designed to support older people with short-term assistance that will help them to improve their health and independence at the end of their hospital stay. The Program provides older people who are eligible with a package of services that include low intensity therapy (such as Physiotherapy, Occupational therapy and/or Social work), Nursing support and/or Personal care. Transition Care provides time-limited support (for up to 12 weeks) and has a therapy focus. The Neighbourhood Centre is a community facility offering a number of services including group activities, childcare, emergency relief, and a resource library with free use of computers. The Chronic Pain Education and Support Group meets the 1st and 3rd Thursday of every month between 10am - 12 noon.
