Albany Lions Community Care Centre is a HACC service provider to the frail aged and people with a disabilty to remain living in the community. The following services are provided :- Assessment - to determine HACC eligibility ; Centre-based Day Care(meals provided ) M-F ; Domestic Assistance-for tasks such as vacuuming, cleaning plus windows and shopping etc. ; Home maintenance - to help keep your home and garden safe , secure and functional.These include repairs or maintenance such as tap washers,smoke alarms, light globes , cleaning gutters , minor carpentry works, clearing pathways, gardening and pruning etc ; Personal Care- showering , grooming and dressing; Carer Respite - in home respite to take the carer's role and give them a break or respite ;Social Support - to assist with tasks such as banking, shopping and providing company for outings etc; Transport- available 7 days a week between 9am and 4pm. Need to book 24hrs in advance.
Provide support to people in their own home who suffer from a mental illness. Have one unit for limited stay (two bedroom house- this is a step-down facility) up to 3 months. Clients are referred through their case managers or through GSMHS. Support activities include: independent living and household management skills; accessing community services to foster development of social skills, confidence and self-esteem. This service mainly provides outreach to people with a mental illness in their own homes, or other community residential/facility settings. In-home support is individualised around the client's own goals and lifestyle with an emphasis in learning and or maintaining skills with a view to recovery. They also offer respite in-home for the carer to have a break.They also have a community sports and recreational program for their clients which is also an out of home respite.
The Personal Helpers and Mentors Program is a new program that supports people whose lives are affected by a mental illness (diagnosed or undiagnosed). Personal Helpers and Mentors will work with eligible clients (16 years and over) to develop goals and strategies to achieve these goals. Umbrella(Inc) provides care and support to frail seniors and people with disabilities from CALD backgrounds. Services provided are :- Domestic Assistance- house cleaning, clothes washing ,shopping and bill paying. Centre Based Day Care Centre and Multicultural Meals- Activities include: excursions, concerts, physiotherapy with music, arts and crafts, picnics and BBQs, guest speakers and movies. Home Maintenance - General repair and care of a client's home or yard , includes : small repairs, lawn mowing, rubbish removal and cleaning gutters. Multicultural HACC program. The delivered programs provide basic maintenance and support services to assist the frail aged, young people with disabilities and their carers to continue to live in the community. Assistance is available with daily activities including showering, shopping, cleaning and preparing meals.

This is an organisation which provides services for residents in the local government area of Claremont, Cottesloe, Peppermint Grove and Mosman Park. These may include basic domestic assistance, personal care, transport, home visiting, minor gardening and home maintenance.
