ACAT assessment must be ticked in Transitional Care box.

Extended Care/Rehabilitation Services This service offers specialised exercise programs and caters for a wide variety of medical and musculo-sketal condiditons as well as for those individuals who have sustained compensable injuries (e.g: workers compensation, MVA and DVA). The service includes hydrotherapy, pilates, physiotherapy, injury rehabilitation and prevention and pre/post surgical rehabilitation. Staff are trained Exercise Physiologists using exercise as a treatment strategy in physical rehabilitation - as a preventative strategy for certain chronic diseases as part of establishing and sustaining functional independence. Staff work with individuals to reach personal goals by providing treatments on a one to one basis or groups.

Palmerston offers a wide variety of residential and non-residential services but this varies in each branch .The Sth Metro CDS provides an extensive range of prevention, treatment and counselling services for people in south metro. The South Metro Community Drug Service is an amalgamation of two services that were operating separately: the South Metropolitan Drug and Alcohol Clinic, managed by Next Step Drug and Alcohol Services, and Palmerston Association's South Metro Community Drug Service Team.
