Provision of expert Medical and Surgical care within the home environment after an initial stay in hospital.

South West Emergency Care For Children

"Short term emergency care" for families with young children where primary carer is absent due to sickness or other crisis and there are no family or friends to assist. Respite for parents who need emergency time out to attend counselling, for hospitalisation etc. Meal preparation, transport available for children where necessary. Assessment mandatory and fees may be charged based on income, this would need to be negotiated. EMERGENCY SITUATIONS ONLY. All services provided have to relate to the care of the child. Service provided to Lower South West only.

Transition Care Service

Provides transitional care between hospital and home for aged hospital inpatients who are stable and motivated. 20 places available of which 5 are dedicated to people with high care needs. Referral from hospital, GP, specialist, ACAT team, social worker, O/T, other allied health personnel. Must have been ACAT assessed in last 4 weeks. ACAT assessment must be ticked in Transitional Care box.
